Home care management options

1. Self-Managed

Your choice of Self-Managed Home Care includes the basic services that we provide listed below, and you can enjoy the full independence to manage your care needs. If you require additional Care Management service, the hourly rate of the Care Management Fee will apply. This choice is suitable to those who are allocated Level 1 or 2 HCPs with good understanding of Government Policy of what can be included in Home Care Packages and exclusion items, and only need basic level of support without case management services.

2. Partnership Care Management

Your choice of Partnership Care Management includes regular review of your care needs by our professional Care Manager at minimum cost. It means the majority of your home care subsidy will be spent on direct care needs or in-home support services. You will enjoy the allocated time from our Care Management Team to work together and discuss about your home care needs and supports.

3. Full Support Care Management

This is the hassle-free choice for you if you enjoy the optimal level of support and case management by our professional Care Management Team. This choice is suitable to those who wants a peace of mind to enjoy the care services coordinated and managed for you.
